Job summary

Qualifications

• ADN or BSN required; Nursing Diploma, if held, requires a High School Diploma as well.
• Experience or trained (required).
• RN license certified by Virginia DHP (and CPR/BLS) is required.

Responsibilities

• Assess patient clinical and psychosocial needs and develop plan of care.
• Collaborate with interdisciplinary team to coordinate care.
• Educate patients and families and document education and discharge plans.
