Job summary

Qualifications

Responsibilities

• Assess patient clinical and psychosocial needs per unit standards.
• Develop and evaluate care plans with patients and families.
• Coordinate care with interdisciplinary team.
• Ensure accurate clinical and quality documentation.
• Support patient throughput and safe transitions.
