Job summary

Centra Health seeks a Clinical Pharmacist Specialist to deliver patient-centered, evidence-based medication therapy in a decentralized care model in Virginia. You will collaborate with physicians, nurses, and the broader care team to optimize outcomes and ensure safe drug use.

You will lead pharmacotherapy evaluations, participate in antibiotic stewardship, mentor students and residents, and contribute to quality improvement initiatives while maintaining regulatory compliance.

Qualifications

• PharmD from an accredited school.
• Three years of clinical pharmacy practice or ASHP PGY1 residency.
• Virginia license to practice; BCPS certification within 1 year if not already held.

Responsibilities

• Consults with medical staff to design drug regimens that minimize toxicity and optimize outcomes.
• Performs comprehensive patient monitoring to identify and mitigate drug-related problems and adverse events.
• Evaluates medication orders for indication, dose, route, and frequency in relation to history and current orders.
• Formulates and adjusts total parenteral nutrition based on patient parameters and monitors daily.
• Provides pain assessments and designs pain management plans; participates in antibiotic stewardship and other restricted programs.
• Delivers direct patient care as part of a multidisciplinary team and serves as a pharmacotherapy resource.
