Norton Healthcare is seeking a Clinical Pharmacist Specialist to deliver evidence-based, collaborative, patient-centered care across our system. The role focuses on optimizing therapy, reducing adverse events, and trimming costs while developing guidelines and educational activities for physicians, nurses, residents, and students.

You will participate in guidelines implementation, perform consultations, monitor adverse events, and contribute to formulary management and performance improvement

Qualifications

• Three years of pharmacy practice or PGY-1 residency/fellowship completed.
• Bachelor degree in pharmacy or related field.
• Licensed pharmacist (Pharmacist).

Responsabilités

• Develop system-wide guidelines and programs to improve practice and outcomes.
• Provide cost-effective, patient-centered medication therapy consultations.
• Collaborate with physicians and health care team; educate staff and students.
• Support QA, performance improvement indicators, and resource budgeting targets.
• Represent the pharmacy department on committees and assist formulary management.
