Job summary

Bernalillo County is hiring a Forensic Case Manager to provide case management for assigned clients, conducting interviews, assessing needs, and coordinating referrals with licensed clinicians, victim liaisons, and law enforcement. The role supports individuals facing addiction, homelessness, and mental health challenges, and reviews cases with law enforcement partners.

The position requires collaboration across agencies, client advocacy, and ensuring timely documentation and reporting.

Qualifications

• Bachelor’s degree or four years in related field required.

Responsibilities

• Interview clients and review case history to determine case management needs.
• Coordinate with community agencies to help clients schedule appointments and access care and services.
• Assess treatment and services and make referrals for behavioral health, medical, legal, or other support.
• Document and report case management activities accurately and on time.
• Prepare case files for periodic review and quality assurance.
• Attend professional meetings and stay current on best practices for the role.

Education

Bachelor’s degree in criminal justice, psychology, social work, counseling, case management or related field

Four (4) years in criminal justice, psychology, social work, counseling, case management or related field
