Job summary

Henry Mayo Newhall Hospital in Valencia, CA seeks an EmPATH Social Worker to join a multidisciplinary team in the crisis stabilization unit. You will assess, plan, and coordinate care for adults with psychiatric conditions, providing biopsychosocial evaluations, therapy, and discharge planning.

The role emphasizes collaboration with ED staff, psychiatrists, and nurses, with responsibilities for group and individual therapy and family engagement.

Qualifications

• Master’s Degree in Social Work required.
• One year of hospital and/or mental health social work experience required.
• Care management experience preferred.
• 2–3 years in Behavioral Health field preferred.
• Current BLS provider card required.
• Current ACSW through BBS required or in progress.

Responsibilities

• The EmPATH Social Worker provides social work services to the adult population in the crisis stabilization unit.
• Coordinates care with the multidisciplinary treatment team (ED providers, psychiatrist, program manager, clinical RNs, staff).
• Performs biopsychosocial and behavioral health assessments and case management.
• conducts treatment and discharge planning, as well as group, individual and family therapy.
• Assists patients and families with social and emotional needs related to psychiatric condition, treatment and recovery.
