Job summary

Rosecrance Health Network in Rockford, IL is seeking a Crisis Stabilization Case Manager to assess and establish appropriate transitional care for clients entering services after a crisis. You will manage caseloads, coordinate with the crisis supervisor and other programs to ensure effective linkages to ongoing care.

Qualified candidates hold a Master’s in a relevant field or QMHP and have crisis response experience, strong communication, and a valid driver’s license.

Qualifications

• Master’s degree or equivalent experience in a related field.
• Must qualify at minimum as a QMHP per Illinois Rule 132.
• Experience supervising a team of behavioral health professionals.
• Skilled in individual and family counseling and crisis interviews.
• Excellent written and oral communication skills.

Responsibilities

• Work scheduled hours as assigned by leadership; some variation may occur.
• Participate in on-call system as required.
• Provide crisis stabilization services in community settings.
• Provide client consultation to crisis response team.
• Counsel and assess clients and guide their needs.
• Complete all clinical documentation in line with regulatory requirements.
• Lead team meetings and provide staff feedback.
• Support leadership and other departmental staff.
