Job summary

Life Consultants Inc. in Chesapeake, VA is seeking a dedicated Human Service Professional to support individuals in transitional housing and crisis homes who struggle with severe mental health challenges and need help locating permanent housing.

The Residential Crisis Stabilization program provides 24-hour supervision in a 5-day step-down from hospital. Shifts are 7am-3pm, 3pm-11pm, and 11pm-7am; candidates with open availability are encouraged to apply.

Qualifications

• Minimum of 2 years of experience in a similar role or degree in Human Services or related field.
• Strong interpersonal and communication skills.
• Ability to work effectively in a team and independently.
• Knowledge of local resources and support services.
• Empathy and compassion for individuals facing challenges.
• Proficiency in documentation and record keeping.
• Applicants must be able to pass background checks.

Responsibilities

• Provide direct support and counseling to individuals facing various challenges
• Assist clients in setting and achieving personal goals
• Develop and implement individualized treatment plans
• Collaborate with interdisciplinary teams to ensure comprehensive care
• Advocate for clients' rights and access to resources
• Maintain accurate and up-to-date documentation of client progress
• Stay informed about current best practices and trends in human services
• Provide supportive counseling/crisis intervention services to individuals as needed
• Rotating between company facilities to include transitional housing and corporate office
• Transport client(s) to appointments and etc
