Qualifications

Responsibilities

• Provides services to individuals and families referred to the program from external agencies, internal programs, or self-referral.
• Engages individuals/families in services and develops individualized service plans and goals.
• Teaches and provides information on topics such as child development, stress management, parenting, and family life as indicated by risk issues.
• Provides linkage to community resources and advocacy or concrete help to receive needed services.
• Performs transportation to medical, legal and social service appointments as needed.
