Job summary

Intermountain Health is seeking a Clinical Social Worker (LCSW/CMHC) for PRN, 12-hour night shifts in Murray, UT. You will assess medical/mental health histories, coordinate care, and provide psychosocial interventions with a multidisciplinary team.

Qualifications

• Master's Degree in Social Work from an accredited institution.
• Current State Licensure as LCSW or LMFT.
• Computer Skills (email, word processing).
• Excellent written and verbal communication skills.

Responsibilities

• Assesses patient medical and mental health history to understand needs.
• Organizes daily work by assessing patient needs and status.
• Diagnoses and treats mental, emotional, and behavioral disorders.
• Performs social work consultation for patients and caregivers.
• Conducts psychosocial assessments.
• Provides crisis bereavement and other social work interventions as needed.
• Develops a plan of care with input from patient and care team.
• Modifies care plans as patient status changes.
• Refers patients to community services or other providers as needed.
• Educates and coordinates with care teams and outside agencies to ensure timely discharge and quality care.
• May credential through medical staff office and peer review as required.
• Provides staff support/debriefing in coordination with EAP after adverse events.
• Advocates for patients and families to ensure effective communication.
