Job summary

Oaklawn Psychiatric Center in South Bend is seeking a Social Worker / Therapist for Crisis Services to assess clients, develop treatment plans, and provide emergency therapy during crises. The position requires licensure-eligible social work credentials (LSW/LMHCA/LMFTA eligible; LCSW/LMHC/LMFT preferred), a diploma with transcripts, and prior social services

Qualifications

• LSW/LMHCA/LMFTA eligible; LCSW/LMHC/LMFT preferred.
• Must provide diploma and official transcripts.
• Three years of experience in social service setting preferred.

Responsibilities

• Assessing and diagnosing clients.
• Creating treatment plans aligned to the client’s goals.
• Providing emergency therapy interventions and crisis risk assessment.
• Providing crisis intervention and assessing risk factors of harm to self or others.
• Referring to relevant services as needed.
• Providing timely electronic clinical documentation.
• Consulting with and supervising other clinicians.
• Collaborating within a multi-disciplinary team.
