Job summary

Oaklawn Psychiatric Center in South Bend is seeking a Crisis Services Nurse (LPN or RN) to work 8a-8p within the South Bend location. The role includes admission screening, health assessments, wound care, vitals collection, and medication administration under center provider orders.

Qualifications

• Active LPN or RN license in Indiana.
• Experience in crisis or behavioral health settings preferred.
• Ability to assess medical stability and health needs.

Responsibilities

• Assess individuals for admission using a medical stability approach.
• Assess physical health, using a brief health assessment.
• Deliver care for minor physical health challenges.
• Collect vitals, medical, and substance use history.
• Treat minor wounds.
• Administer medication with orders from the center provider.
• Facilitate escalation of care when clinically indicated.
• Help develop and refine center policies and procedures.
• Document all necessary information in patient charts.
