Job summary

Highland Rivers Health is seeking a Community Response Behavioral Health Therapist to partner with the County Sheriff's Office and the Community Services Board on a community response team. You will assess potential mental health and/or substance use disorders, de-escalate crises, and link individuals to ongoing treatment, including 1013/2013 transport when needed.

Qualifications

• Experience in behavioral health crisis assessment and intervention.
• Ability to coordinate with sheriff's office and community services board.
• Knowledge of 1013/2013 processes for emergency transport.
• Strong documentation and reporting skills.
• Ability to educate community partners and stakeholders.

Responsibilities

• Respond in the community to perform behavioral health assessment and intervention for 911 callers.
• Work with the community response team on active 911 calls with mental health concerns.
• Initiate 1013/2013 for transport for emergency evaluation when needed.
• Assess immediate needs and link individuals to appropriate resources.
• Identify social and community service options and make recommendations.
• Collaborate with the team case manager to manage a caseload of wraparound services.
• Develop person-centered behavioral health plans with the team.
• Coordinate follow-up contacts post-crisis.
• Attend staffing and case meetings as required.
• Maintain documentation standards and required records.
• Educate community partners about available programs and services.
• Participate in community events to raise awareness of services.
