Qualifications

• Bachelor’s degree in a mental health discipline.
• Must be 23 years of age or older; has and maintains a valid Alabama driver’s license with a good driving record and must have proof of current liability insurance.

Responsibilities

• Responds to crisis calls in a timely manner
• Makes face to face visits when calls come into CarePointe or the Outreach Liaison
• Provides services in the community, consumer’s homes, etc.
• Demonstrates the ability to recognize the elements of a crisis state and knows how to deescalate or resolve the situation
• Communicates effectively with CarePointe to assist in finding appropriate placement when necessary
• Travels to consumer’s homes to ensure medication compliance as needed
• Documents in a timely fashion per Alta Pointe policy
• Meets Alta Pointe productivity standards
• Takes part in Performance Improvement activities
• Attends required in-service training and workshops
