Job summary

CPC Integrated Health in Eatontown, NJ is seeking a Crisis Worker to provide crisis intervention and de-escalation. The role includes reviewing cases with supervisors, coordinating care with treatment programs, and maintaining client records.

Qualifications

• Bachelor's Degree in Human Services or healthcare related field.
• Minimum 2 years' experience providing treatment services to children or adults.
• Master's Degree in Human Services or healthcare related field plus 1 year prior experience preferred.

Responsibilities

• Deliver Crisis Response services 24 hours a day, 7 days a week. Work a combination of day, evening, and weekend shifts.
• Due to unpredictable crisis response timing, may work past shift end to ensure a safe response and complete assessments as required by state regulations.
• Ongoing reassessment of assigned clients to identify new service needs and follow-up on crisis outreaches.
• Timely and competent completion of progress notes and related documentation; participate in treatment planning with clients and families.
• Coordinate care with treatment programs and provide care coordination, safety planning, and case management.
• Meet clients in homes, schools, hospitals, and other community settings.

Education

Bachelor's Degree in Human Services or healthcare related field

Master's Degree in Human Services or healthcare related field plus 1 year prior experience
