Job summary

YAI is seeking a Social Worker to provide crisis intervention services for individuals with I/DD and their families in New York City. You will conduct intakes, coordinate eligibility for OPWDD services and connect clients to needed supports.

The role involves on‑site and in‑person work, crisis counseling, and discharge planning with supervision from a clinical team. Strong communication and organization are essential.

Qualifications

• Master’s degree in Social Work required.
• Familiarity with intellectual/developmental disabilities (I/DD) and behavioral/mental health needs.
• Excellent communication and interpersonal skills, written and verbal.
• Able to prioritize tasks and manage caseloads in a collaborative environment.

Responsibilities

• Manage an assigned caseload of individuals with I/DD receiving crisis intervention services and coordinate access to supports.
• Review referrals and inquiries, determine eligibility, and schedule intake meetings as needed.
• Conduct in‑person intake meetings and home visits to assess service needs and coordinate evaluations.
• Provide ongoing crisis counseling and behavior management under clinical supervision; educate families.
• Assist with discharge planning and transitions to long‑term services in collaboration with the team.
