Qualifications

• Bachelor's degree in Human Services or related field.
• Knowledge of mental health, substance use, crisis intervention, and community-based services.
• Ability to work independently in a fast-paced, community-based environment.
• Valid Texas driver's license and approved driving status.

Responsibilities

• Engage individuals experiencing homelessness or housing instability.
• Conduct behavioral health, safety, housing, and service-needs assessments.
• Provide crisis intervention, de-escalation, safety planning, and follow-up.
• Develop and monitor recovery and housing stability plans.
• Coordinate housing, benefits, medical care, behavioral health treatment, and referrals.
• Assist with obtaining identification, benefits, and housing documentation.
