Job summary

Bluebonnet Trails Community MHMR Center seeks a Crisis Clinician to provide outreach, crisis intervention, case management, and housing-focused support to individuals and families experiencing homelessness or at risk of homelessness.

You will conduct assessments, create recovery and housing stability plans, and connect clients with behavioral health, medical, housing, benefits, and other community resources.

Qualifications

• Bachelor’s degree in Human Services or a related field.
• Master’s degree in Human Services or a related field (preferred).
• Knowledge of mental health, substance use, crisis intervention, and community-based services.
• A valid Texas driver’s license and approved driving status are required.

Job description

Under the supervision of the Director and Program Manager, the Project HOPE Crisis Clinician provides outreach, crisis intervention, case management, and housing-focused support to individuals and families experiencing homelessness or at risk of homelessness. The clinician conducts assessments, develops person-centered recovery and housing stability plans, provides short-term crisis stabilization, and connects individuals with behavioral health, medical, housing, benefits, and other community resources.

The Crisis Clinician works in community-based settings and collaborates with law enforcement, hospitals, shelters, local governments, schools, community organizations, landlords, and BTCS treatment teams. Services are recovery-oriented, trauma-informed, and focused on improving safety, stability, and access to permanent housing.

The clinician maintains timely and accurate electronic documentation, monitors progress toward program goals, and assists with collecting required grant and outcome data. This position requires the ability to work independently in the community, prioritize changing needs, and maintain a flexible schedule that may include occasional evenings or weekends.

Key Responsibilities

• Conduct outreach and engage individuals experiencing homelessness or housing instability.
• Complete behavioral health, safety, housing, and service-needs assessments.
• Provide crisis intervention, de-escalation, safety planning, and follow-up services.
• Develop and monitor person-centered recovery and housing stability plans.
• Provide case management, resource coordination, and referrals for housing, benefits, medical care, behavioral health treatment, transportation, and other supportive services.
• Assist individuals with obtaining identification, benefits, housing documentation, and other items needed to secure and maintain housing.
• Coordinate short-term emergency or interim housing placements when authorized by the program.
• Collaborate with law enforcement, hospitals, community agencies, landlords, families, and BTCS programs.
• Maintain accurate and timely electronic documentation and required grant data.
• Participate in team meetings, training, community outreach events, and quality-improvement activities.
• Perform other duties necessary to support Project HOPE operations and contractual requirements.

Location: Primary Counties: Guadalupe, Caldwell, Gonzalez (Southern Hub) | Secondary Counties: Bastrop, Fayette, Lee (Central Hub)

Hourly Salary Range: $24.23 – $31.48

• $1.00 per hour – 2 year previous experience
• Up to $4.00 per hour – BTCS Tenure
• $0.75 per hour – Bilingual
• $1.00 per hour – Advanced Licensure
• $0.50 per hour – LCDC

Credentials Minimum Qualifications:

Bachelor’s degree in Human Services or a related field.

Knowledge of mental health, substance use, crisis intervention, and community-based services.

Candidates must be able to work independently and collaboratively in a fast-paced, community-based environment and effectively engage individuals with complex needs.

A valid Texas driver’s license and approved driving status are required.

Candidates whose degree is not in Human Services must have completed at least 24 credit hours in a human services-related field.

Preferred Qualifications:

Master’s degree in Human Services or a related field.

Experience providing crisis intervention, homeless outreach, housing navigation, or community-based case management.

Pursuit or possession of clinical licensure in counseling, social work, marriage and family therapy, behavior analysis, or a related discipline.

Bilingual proficiency in Spanish or ASL.

Experience with concurrent electronic documentation.

Current licensure as a Licensed Chemical Dependency Counselor.
