Job summary

Responsibilities

• Provide crisis intervention, referrals and linkage to community resources.
• Utilize supportive techniques and appropriate educational methods.
• Complete service documentation accurately and timely in the electronic health record.
• Provide prevention and education services.
• Facilitate referrals to appropriate level of care.
• Develop and implement realistic goals and intervention plans.
• Collect, track and evaluate program data for fidelity and service quality.
• Adhere to professional standards as outlined by protocols and regulations.
• Other duties as assigned.
