Qualifications

Responsibilities

• Identify and assess callers' needs.
• Provide crisis intervention, referrals and linkage to community resources.
• Utilize supportive techniques and/or educational methods appropriate to caller and topic.
• Complete service documentation accurately and timely in the electronic health record.
• Provide both prevention and education services.
• Facilitate referrals to appropriate level of care.
• Develop, define and implement realistic goals and plans for interventions.
• Collect, track and evaluate program data for fidelity and service quality.
• Adhere to professional standards as outlined by protocols, rules and regulations.
• Other duties as assigned.
