Job summary

Mount Rogers is seeking a Crisis Counselor I to provide clinical services in the community, crisis stabilization center, home, and through mobile outreach in Wytheville, VA. The role requires delivering rapid assessments, planning, and coordinating care across multiple systems.

You will document all encounters in the electronic health record, develop individualized service plans, and work with a multidisciplinary team to help individuals stabilize and function in the community.

Qualifications

• Bachelor’s degree in social work, psychology, or related field.
• QMHP-Eligible required; QMHP with experience preferred.
• Valid driver’s license with clean record.

Responsibilities

• Provide crisis stabilization in the crisis care center, home, and community through mobile services.
• Develop effective and measurable individualized service plans for crisis stabilization.
• Coordinate services and interventions across multiple systems to stabilize clients.
• Document all encounters in the electronic health record (EHR) and complete required reports.
• Collaborate with team and community partners to support ongoing care.
