Description

Position Overview: This is an as-needed position within the In-House Registry (IHR). No benefits are provided. Availability is needed to cover both day and night shifts, weekdays and weekends. The Case Therapist I reflects the mission, vision, and values of Northwestern Medicine (NM), adheres to the organization's Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines, and regulatory standards.

Responsibilities include:

  • Assessing patients for acute mental health crises and substance concerns in emergency and medical/surgical units.
  • Developing treatment plans and providing direct patient services, including individual and family counseling as needed.
  • Coordinating care by communicating with treatment teams, patients, and families, documenting in medical records, and discharge planning.
  • Providing accurate information and timely updates to patients and customers, addressing questions and concerns promptly.
  • Demonstrating teamwork, effective communication, and respect for diverse opinions and styles.
  • Managing work schedules efficiently to complete tasks timely.
  • Documenting in electronic medical records accurately and promptly.
  • Understanding DSM diagnoses and developing therapeutic interventions through clinical contact and input from the treatment team.
  • Communicating regularly with patients and support systems regarding admissions, stays, and discharges, and documenting these communications.
  • Completing evaluations and assessments as part of intake, justifying recommended levels of care.
  • Responding to shifts in patient needs and remaining on duty during crises; accepting on-call work as required.
  • Building and maintaining good rapport with physicians, departments, and staff.
  • Meeting departmental expectations regarding patient caseloads, quality of care, and patient satisfaction.

Qualifications:

Required:

  • Experience with substance abuse and mental health populations.
  • IL LPC or LSW Licensure.
  • Masters degree in social work, psychology, counseling, or a related field.
  • Experience and training in assessment, individual, family, and group counseling.
  • Proficiency with computer systems and electronic medical records.

Preferred:

  • 2-5 years in intensive or mental health treatment settings.
  • Additional licensures such as IL LIC CPC, LIC LCSW, LIC MSW, or certifications like ERT CADC.
  • Presentation skills including lectures and didactic presentations.
