Job summary

Connections Health Solutions is seeking a RISE Case Manager to connect clients to resources supporting stabilization related to homelessness, mental health, and substance use disorders. You will engage with clients and families, coordinate crisis and outpatient services, and collaborate with a multidisciplinary team and law enforcement to develop individualized care plans.

You will document services in the medical record, participate in supervision, and support discharge planning.

Qualifications

  • High School Diploma or equivalent.
  • Experience in a community based or crisis healthcare setting.
  • CPR/First Aid certified.
  • Valid driver’s license.
  • Arizona Level I Fingerprint Clearance Card.
  • Fully vaccinated or exempt per policy.
  • Own vehicle in good condition.

What You'll Do:

The primary focus of the RISE Case Manager is to connect clients with resources and services that support stabilization related to homelessness, mental health, substance use disorders, and primary health needs. The Case Manager engages clients and their families in the community, facilitates connections to crisis and outpatient services, and coordinates access to appropriate community resources. Working collaboratively with the multidisciplinary treatment team and Law Enforcement, the RISE Case Manager responds to crisis situations and develops individualized plans of care utilizing expertise in counseling and crisis intervention to support positive health outcomes for clients and their families.

  • Intervene in community crisis situations with patients and families in the community, and with Law Enforcement.
  • Assess, document, and address psychosocial, cultural, emotional, and economic needs for individuals and families during their immediate need/crisis situation.
  • Collaborate with the multi-disciplinary treatment team, outpatient care team, and community agencies as needed to creatively address and/or resolve the crisis situation.
  • Identify and address gaps in service needs for participating parties' in-service delivery to the individual and make appropriate recommendations to meet individuals' needs upon discharge.
  • Collaborate with interdisciplinary treatment team to engage, monitor, and communicate with the individual ensuring that safety is maintained while meeting the individual’s needs.
  • Maintain the clinic record, including documentation of activities performed as part of the service delivery process. Obtain signatures from individual regarding treatment, as necessary.
  • Document all services and activities in the medical record regarding brief interventions, coordination of care, discharge planning, treatment planning, etc. Ensure documentation in the Electronic Health Record (EHR) is clear, concise, and organized.
  • Maintain a working knowledge of the Title 36 Involuntary Commitment process. Monitor the petition process to ensure timeliness is adhered to, and forms are complete and thorough, and when subpoenaed, attend COE hearings.
  • Participate in individual and group supervision as required. Participate in ongoing education, including in-services, training, and other activities to maintain and improve competency. Demonstrate competency through post-testing, skill observation, and performance as assessed by direct supervisor.
  • Assist individuals and families in making difficult decisions, while honoring their preferences and values, in order to move the discharge plan forward.
  • Conduct re-engagement field visits with individuals who have an established relationship and treatment plan but are no longer actively engaged in services, with the goal of reconnecting them to appropriate care and support.
  • Respond as a second responder to complex crisis events in partnership with participating law enforcement agencies, providing case management, peer support, and connections to appropriate behavioral health resources.
  • Conduct follow-up visits, as requested by law enforcement agencies, with individuals who have experienced an overdose, prioritizing follow-up within the best-practice standard of 24-72 hours, based on agency capacity and caseload.
  • Co-locate at assigned law enforcement stations and maintain availability to support officers and community members during designated hours.
  • Participate in community forums and outreach activities to support the RISE program and community efforts to reduce overdose deaths.
  • Serve as a liaison among law enforcement agencies, Justice Services Liaisons, and treatment providers to coordinate services and ensure the needs of targeted individuals are appropriately addressed.
  • Performs all other duties as assigned.

  • Must have own vehicle that is clean, neat, and in good working condition
  • Willing to obtain Full coverage car insurance with high-level liability
  • Arizona Level I Fingerprint Clearance Card

It would be great if you had:

  • Bachelors Degree in Behavioral Health or related field
  • 2 years of Behavioral Health Experience in a community based or crisis healthcare setting
  • Knowledge of evidence-based/person-centered skillsets, such as motivational interviewing, trauma informed care, verbal de-escalation, harm reduction, and person-centered language
  • Experience using EHR
