Job summary

Spurwink seeks a dedicated clinician to provide 14+-focused crisis assessments and care in a 24/7 per diem role. You will maintain records, oversee ISP implementation, coordinate with community partners, and support families through in-person and outreach visits.

The position requires MHRT-C certification or eligibility (or equivalent), a valid driver's license, and a bachelor's degree. Competitive benefits and flexible scheduling are offered in a mission-driven setting.

Qualifications

• MHRT-C certification or MHRT-C eligible, or a Certified Rehab Counselor or Employment Specialist or C or Licensed Alcohol and Drug Counselor.
• Bachelor's degree from an accredited four-year institution preferred.
• Must maintain a current driver's license and clean driving record as per Spurwink Driving Policy.

Responsibilities

• Maintain client records and documentation per Spurwink standards including assessments, ISP, progress notes, and reviews.
• Oversee implementation of Individual Service Plan and monitor progress.
• Maintain regular contact with team members and other professionals involved with the client.
• Identify and assess physical and behavioral health risk factors and ensure awareness of community services.
• Provide support via phone, home visits, and community outreach.
