Job summary

Connections Health Solutions is seeking a full-time Case Manager for their RESET Center in San Francisco. The role provides vital support in behavioral health crisis care, focusing on person-centered engagement and community resource connections.

Successful candidates will have 1-3 years of relevant experience and must possess a High School diploma. The position pays between $26 - $30 USD per hour along with comprehensive benefits including health insurance, PTO, and 401k matching.

Qualifications

• 1-3 years of experience in harm reduction, crisis services, or related community care settings.
• Must obtain Overdose Prevention and Narcan Administration training within 30 days of hire.
• Ability to successfully complete Basic Life Support (BLS) certification during the first week of employment.

Responsibilities

• Provide person-centered care coordination and engagement to individuals utilizing the sobering program.
• Conduct brief screenings, engage in crisis de-escalation, and collaborate with community partners.
• Develop short-term, accessible discharge plans for individuals staying between 4–24 hours.
