Screen requests for services to determine the problem(s), assess risk level, and prioritize needs.
Provide crisis intervention and resolution, including symptomatic relief and referrals.
Facilitate admission to AltaPointe program services ensuring appropriate level of care.
Document in EMR accurately and timely according to policy.
Coordinate follow-up services and provide information on resources.
