Job summary

Connections Health Solutions seeks a Licensed Practical Nurse (LPN) to join our crisis care team in a full-time capacity. You will administer and document medications, transcribe orders, and conduct continual assessments while collaborating with a registered nurse to implement care plans.

Responsibilities include patient education on behavioral health diagnoses, safety-focused interventions, and supporting the care team in a high-paced environment.

Qualifications

• Graduation from an accredited Nursing Program.
• An unrestricted, active LPN license.
• 2 years of experience in psychiatric/behavioral health.

Responsibilities

• Nursing Support and Collaboration: assist RN, contribute data for assessments and health history.
• Medication Administration and Documentation: transcribe orders, administer meds, ensure reconciliation.
• Clinical Tasks: draw labs and provide wound care as ordered.
• Behavioral Health Support: educate patients and families on diagnoses and medications.
• Crisis Intervention: defuse dangerous situations using appropriate verbal techniques.
• Emergency Response: administer emergency meds during seclusion/restraint under RN supervision.
• Additional Duties: perform other duties to support team.
