Job summary

Qualifications

• Bachelor's degree required; experience with diverse health needs preferred.
• CRC credential required.
• Ability to comply with HIPAA, Person First standards, and organizational policies.

Responsibilities

• Review FCOs and supporting docs to prepare for client assessments.
• Conduct vocational assessments and reassessments within required timelines.
• Develop IVAPs, collect data, identify barriers, and outline next steps.
• Discuss clients' employment goals using vocational tools.
• Provide resources and referrals and coordinate with team members.
• Maintain detailed client records and support high-volume flow.
