Qualifications

• Strong clinical assessment and counseling abilities.

Responsibilities

• Evaluate behavioral, emotional, cognitive, spiritual, and social needs of clients and their families.
• Develop, implement, and monitor individualized treatment plans with the treatment team.
• Facilitate individual counseling, family therapy, groups, and psychoeducational programming.
• Coordinate care with referral sources, outpatient providers, and stakeholders.
• Maintain accurate clinical documentation in the EHR per policies and standards.
• Participate in discharge planning, aftercare development, and outcome tracking.
