Job summary

Stone River Recovery Center is seeking a Counselor I (PRN) in San Antonio, TX. The role delivers treatment support for clients recovering from addictions and co-occurring disorders, including family involvement when appropriate.

Responsibilities include conducting assessments, documenting care, and coordinating care with referrals and insurers. A Bachelor's in a health/social field is required, with licensure or eligibility and CPR/de-escalation training as needed.

Qualifications

• Bachelor's degree in social or health services required.
• Licensure or certification for counseling as required by the state; eligible within designated timeframe.
• CPR and de-escalation certification required (training available).

Responsibilities

• Participates in client treatment planning and continuum of care.
• Conducts group and/or family sessions under supervision and documents accordingly.
• Prepares individual treatment plans and initial/follow-up assessments.
• Acts as liaison between referral sources and clients and coordinates with insurance carriers.
• Provides crisis intervention and plans for aftercare.
• Carries out case management duties to ensure individualized care and coordination with other programs.

Counselor-PRN | Stone River Recovery Center | San Antonio, TexasAbout the Job:LCDC preferred! The Counselor I provides treatment and support to assist clients in their recovery from addictions, such as alcoholism, drug addiction, eating disorders, or other behavioral problems, assists with modifying problem behaviors; may also provide counseling services to families of clients.Roles and Responsibilities:· Participates in client treatment planning and continuum of care.· Conducts group and/or family sessions, under supervision, as scheduled and documents as required.· Co-facilitates assigned group or family sessions and assists with modifying problem behaviors.· Responsible for initial assessments, as well as follow up assessments for clients.· Prepares individual treatment plan for each assigned client.· Ensures all documentation regarding client care, treatment and incidents is completed timely and in a clear, concise manner.· Acts as a liaison between referral sources and clients.· Provides crisis intervention to clients, as needed.· Plans for aftercare for assigned clients.· Acts as a liaison with insurance carriers, as requested.· Provides case management duties for clients, ensuring individualized quality care.· Evaluates the need of the client and determines if referrals to other programs or facilities is needed.· Provides client and family education relative to substance abuse/psychiatric problems.· Conducts educational workshops both within the Facility and in the community as required.· Serves as a liaison with self-help groups and EAP professionals.· Conducts ongoing assessments of chemical dependence services required by the Facility treatment programs.· Bachelor's degree in social or health services field required.· Experience in counseling field instruction/internship or similar practicum through education or certification process required.· Licensure or certification for counseling, as required by the state in which the facility operates; OR state license or certification eligible within the timeframe designated by the facility.· CPR and de-escalation certification required (training available upon hire and offered by facility).· First aid may be required based on state or facilityWhy Stone River Recovery Center?Stone River Recovery Center offers a comprehensive benefit plan and a competitive salary commensurate with experience and qualifications.
