Job summary

Spring Creek Rehab and Nursing Center is seeking a Certified Occupational Therapy Assistant (COTA) to join our dedicated team in Harrisburg, PA. This full-time role offers a flexible schedule with PRN opportunities and a supportive, patient-centered environment.

Your scope includes planning and delivering OT services, assessments, and collaboration with the care team to meet resident goals while maintaining compliance with standards.

Qualifications

• Associate degree required for COTA position.
• Must pass post offer background check and drug screen.
• Ability to lift 5–45 lbs intermittently.

Responsibilities

• Assist in planning, organizing, developing and delivering OT services per plans and standards.
• Provide individual therapy to residents per care plans.
• Assist with resident assessments and therapy plan development with MDS.
• Coordinate therapy appointments with other departments and discharge planning.
• Maintain equipment and therapy area cleanliness.
