Job summary

Supplied Talent is seeking a Certified Occupational Therapy Assistant (COTA) to join a well-established skilled nursing facility in Las Animas, Colorado. You will work with a collaborative rehabilitation team to enhance residents' independence and quality of life in a close-knit, community-focused setting.

The ideal candidate is a graduate of an accredited OTA program with NBCOT certification, an active Colorado OTA license (or eligibility), and CPR/BLS certification.

Qualifications

• Graduate of an accredited Occupational Therapy Assistant program.
• Current NBCOT certification (COTA).
• Active Colorado OTA license (or eligibility/reciprocity).
• CPR/BLS certification (or ability to obtain upon hire).
• New grads welcome; SNF or long-term care experience a plus.
• Strong documentation skills and a team-first, resident-centered mindset.

Responsibilities

• Provide occupational therapy treatments under supervision and plan of care established by a licensed OT (OTR/L).
• Help residents improve or restore ADLs, fine motor function, cognitive tasks, and safe mobility.
• Document treatment sessions, progress, and outcomes in the EMR (Point of Care/PCC).
• Communicate resident progress and concerns to the supervising OT and care team.
• Educate residents, families, and caregivers on adaptive techniques and home exercise programs.
• Support discharge planning and participate in care plan meetings as needed.
• Maintain compliance with state, federal, and facility regulations including Medicare/Medicaid documentation standards.
