Hayward Gardens Post Acute in Hayward, CA is seeking a compassionate Certified Occupational Therapy Assistant (COTA) to join our in-house rehabilitation team. You will provide hands-on OT under the supervision of a licensed therapist, helping residents regain independence and improve daily living skills.

The role emphasizes ADL training, individualized treatment plans, and collaborative care with an interdisciplinary team.

Qualifications

• Current and active COTA license in California.
• Graduate of an accredited Occupational Therapy Assistant program.
• Experience in skilled nursing or geriatric rehabilitation preferred.
• Strong communication, organization, and clinical skills.
• Ability to work independently while collaborating with an interdisciplinary team.
• Passion for providing resident-centered care.

Responsabilités

• Provide skilled occupational therapy treatments under the direction of a licensed Occupational Therapist.
• Assist residents in improving their ability to perform activities of daily living (ADLs).
• Implement individualized treatment plans designed to improve strength, coordination, balance, mobility, and functional independence.
• Monitor resident progress and communicate updates to the supervising Occupational Therapist.
• Maintain timely, accurate, and compliant clinical documentation.
• Collaborate with nursing staff, physicians, residents, families, and interdisciplinary team members.
• Participate in Resident Care Conferences and rehabilitation meetings as needed.
• Assist with training Restorative Aides and supporting restorative nursing programs.
• Educate residents and families on adaptive techniques, safety, and strategies to promote independence.
• Maintain a clean and safe treatment environment while following infection control and equipment safety procedures.
