Job summary

Continuum Therapy Partners in Minnesota is seeking a Certified Occupational Therapy Assistant to join Heritage Manor-Chisholm on a PRN basis. The role supports therapists by implementing treatment plans and monitoring patient progress under supervision.

Qualified candidates will hold an Associate Degree in an OTA program, current MN COTA license, and NBCOT certification. One year of experience is preferred, and excellent communication with patients and care teams is essential.

Qualifications

• Associate Degree in Occupational Therapy Assistant Program required.
• Current, unencumbered state COTA License; NBCOT Certification required.
• 1 year of previous experience as a Certified Occupational Therapy Assistant preferred.

Responsibilities

• Implements individualized treatment plans under the supervision of an Occupational Therapist.
• Documents patient progress and reports changes in patient status to the supervising OT.
• Educates patients on adaptive equipment and home exercise programs.
