Job summary

Evolve Therapy Services in Tupper Lake, NY seeks a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility. This role offers Full Time, Part Time, or PRN options with a sign-on bonus.

As an OTA, you will implement therapy plans, coordinate with the supervising therapist, document services, and collaborate with residents and families to support quality care. Active OTA licensure and ongoing education are required.

Qualifications

• Associate's degree in Occupational Therapy is required.
• Active OTA licensure in state(s) of practice is required.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Screen residents to determine appropriate intervention.
• Implement treatment plans and goals with supervision from the licensed therapist.
• Communicate evaluation findings, treatment plans, and recommendations to residents, families, and the interdisciplinary team.
• Establish individual resident therapy schedules with the team.
• Deliver therapy per plan and ensure proper documentation and billing of OT services.
• Perform other duties as assigned to effectively treat patients.
• Adhere to CMS documentation standards in daily practice.
