Job summary

Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility located in Tupper Lake, NY. This full-time role offers a sign-on or relocation bonus, with part-time or PRN options as well.

Candidate must have an Associate's degree in Occupational Therapy and an active OTA licensure in the state(s) of practice, plus ongoing continuing education to maintain licensure. The position provides competitive pay and benefits based on status.

Qualifications

• Associate's degree required.
• Active OTA licensure in state(s) of practice required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine best intervention or treatment.
• Perform treatment plans according to established goals and plan of care.
• Interpret and communicate evaluation findings and recommendations to residents and families.
• Establish individual resident therapy schedules with team members.
• Ensure documentation and billing of occupational therapy services in accordance with the plan of care.
• Perform other duties as assigned to effectively treat patients.
• Comply with CMS documentation standards daily with point-of-care documentation.
