Job summary

Three-Meadows-Post-Acute is seeking an Occupational Therapy Assistant to work alongside our Staff OT, delivering hands-on resident care to help regain independence.

You will build meaningful relationships with residents and families, document progress, and contribute to discharge planning while educating on home programs and equipment needs. Join a collaborative team focused on quality care and program development.

Qualifications

• Associate degree from an accredited COTA program.
• Active state licensure as an Occupational Therapy Assistant in good standing.
• CPR certification as required by state licensure.

Responsibilities

• Provide direct resident treatment under OT guidance.
• Build real relationships with residents, families, and the care team; communicate progress, goals, and discharge plans.
• Educate residents and families on home exercise programs, adaptive equipment, and fall prevention.
• Recommend and help order equipment to restore independence (splints, assistive devices, grab bars).
• Help train aides and provide in-services to staff to share expertise.
• Participate in Resident Care conferences and contribute to program development and quality initiatives.
