Blue Stone Therapy is seeking an Occupational Therapy Assistant (COTA) to provide rehabilitation services to adults and seniors at Prairie View Senior Living in Tracy, MN. The COTA works under the supervision of a licensed Occupational Therapist to implement plans of care aimed at improving ADLs, mobility, strength, and independence.

The role requires graduation from an accredited OTA program, MN licensure or eligibility, and prior experience in SNF or post-acute rehab.

Qualifications

• Graduate of an accredited Occupational Therapy Assistant program.
• Current OTA license or eligibility for licensure in the applicable state.
• Experience in skilled nursing, long-term care, or post-acute rehabilitation preferred.
• Strong communication, organization, and clinical reasoning skills.
• Commitment to patient-centered care and professional standards.

Responsabilités

• Provide skilled OT treatment per established plans of care.
• Implement individualized programs focusing on functional outcomes.
• Treat adult and geriatric patients in SNF and rehabilitation settings.
• Monitor patient response and communicate progress with supervising OT.
• Maintain timely documentation to support quality care and reimbursement.
• Collaborate with interdisciplinary team and facility staff to support patient outcomes.
