Chatham Hills Subacute Care Center in Short Hills, NJ is seeking a Certified Occupational Therapy Assistant (COTA) to support the rehabilitation team under the supervision of an Occupational Therapist. The COTA will implement treatment plans, document progress, and collaborate with multidisciplinary staff to deliver patient-centered care in a long-term care setting.

Qualifications

• Must have a valid professional license and registration for state of practice.
• Provide comprehensive treatment to patients under supervision of Occupational Therapist.
• Experience with long-term care and/or geriatric care is preferred.
• Well versed with documentation writing.
• Document patient’s progress on weekly progress note in a timely fashion.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence.
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation

Responsibilities

• Carry out OT care plans under OT supervision.
• Deliver treatment aligned with goals set by the OT.
• Maintain timely progress notes and documentation.
• Collaborate with therapy and nursing teams for patient-centered care.
• Demonstrate professionalism and punctuality in all shifts.
• Perform additional duties as assigned by Rehabilitation Director.
