Job summary

Flagstar Nursing seeks a Certified Occupational Therapist Assistant (COTA) for Skilled Nursing Facilities in NYC and Long Island. You will work with a multi-disciplinary team to provide therapies, educate patients and families, and help improve daily functioning.

We offer direct employment or as an Independent Contractor with corporation setup assistance. Pay ranges $34–$36/hr (W-2) or $38–$40/hr (1099), with flexible full/part-time schedules and weekly pay.

Qualifications

• Active NYS OTA license and registration required.
• EMR proficiency and ability to document progress.
• Immunizations and medical documentation for SNF work.
• Valid proof of identity and work authorization.
• Malpractice/liability insurance required.

Responsibilities

• Collaborate with OT Supervisor to follow Plan of Care for patients.
• Provide quality therapy services in SNF for conditions like TBI, CVA, stroke, Parkinson's, MS, cancer, orthopedic surgery.
• Monitor and document patient progress using EMR.
