Job summary

Liberty Center is seeking a certified COTA to join our rehabilitation team in a full-time or part-time capacity. The Occupational Therapist Assistant works under an OTR to provide patient-centered treatment in accordance with state guidelines.

Responsibilities include patient progress documentation, ensuring MD orders for treatments, and collaborating with the rehab team in meetings. The role requires active state licensure as a COTA and a commitment to quality care in a skilled nursing setting.

Qualifications

• Must have current COTA license.

Responsibilities

• Provide comprehensive treatment to patients under the supervision of an OTR and according to state guidelines.
• Assist OT in identifying appropriate patients and referring them to rehab accordingly.
• Document patient's progress on weekly progress note in an accurate and timely fashion, and obtain the co-signature of the OTR.
• Ensure MD orders are obtained for treatments, recertification, and discharges.
• Assist with other documentation requirements as indicated and according to state guidelines.
• Attend facility and rehab meetings when indicated by the supervisor.
• Perform all other duties assigned by the department supervisor, lead therapist, or regional manager
