Job summary

Linwood Meadows Care Center in Visalia, CA is seeking a full-time Certified Occupational Therapy Assistant (COTA) to join a compassionate rehab team. The candidate will work under the supervision of an OT to provide rehabilitative services and help residents regain independence in daily living and mobility.

The role requires an accredited OTAS program degree and CA licensure; experience in skilled nursing is preferred.

Qualifications

• Associate’s degree from an accredited OTAS program.
• Current COTA license in good standing with CA.
• Experience in skilled nursing or post-acute care preferred.

Responsibilities

• Provide direct patient care per treatment plan.
• Assist in implementing treatment goals and plans.
• Monitor patient progress and report changes to OT.
• Document treatments and responses using Casamba/PointClickCare.
• Educate residents, families, and staff on therapy goals and home programs.

Salary range: $40-$45/hour BOE

Position Summary:

The Certified Occupational Therapy Assistant works under the supervision of a licensed Occupational Therapist to provide rehabilitative services to residents, helping them regain independence in activities of daily living and functional mobility.

Key Responsibilities:

• Provide direct patient care as outlined in the treatment plan developed by the Occupational Therapist
• Assist in the implementation of treatment goals and plans
• Monitor patient progress and report changes to the Occupational Therapist
• Accurately document treatments and patient responses using facility systems (e.g., Casamba, PointClickCare)
• Educate residents, families, and staff on therapy goals and home exercise programs
• Participate in team meetings, care planning, and discharge planning as needed
• Ensure compliance with federal, state, and facility regulations and policies

Qualifications:

• Associate’s degree from an accredited Occupational Therapy Assistant program
• Current COTA license in good standing with the State of California
• Experience in a skilled nursing or post-acute care setting preferred
• Strong communication and interpersonal skills
• Proficient in documentation software and electronic health records
