Job summary

Linwood Meadows Care Center in Visalia, CA is seeking a Full-Time Certified Occupational Therapy Assistant (COTA) to join a compassionate rehab team. Work under the supervision of an OT to help residents regain independence in daily activities and mobility.

The role requires an Associate’s degree as a COTA and a current CA license. SNF/post-acute care experience is preferred. Competitive hourly pay and full benefits available to full-time staff.

Qualifications

• Associate’s degree from an accredited Occupational Therapy Assistant program.
• Current COTA license in good standing with the State of California.
• Experience in a skilled nursing or post-acute care setting preferred.
• Strong communication and interpersonal skills.
• Proficient in documentation software and electronic health records.

Responsibilities

• Provide direct patient care as outlined in the treatment plan developed by the Occupational Therapist.
• Assist in the implementation of treatment goals and plans.
• Monitor patient progress and report changes to the Occupational Therapist.
• Accurately document treatments and patient responses using facility systems (e.g., Casamba, PointClickCare).
• Educate residents, families, and staff on therapy goals and home exercise programs.
• Participate in team meetings, care planning, and discharge planning as needed.
