Job summary

State of Washington, Department of Corrections, seeks a Psychiatric Social Worker 3 for Airway Heights Corrections Center in Airway Heights, WA. The role is full-time and non-permanent, with a typical 8:00am–4:30pm schedule, Monday–Friday.

Responsibilities include transition planning, coordination of care with community resources, NUMERIOUS MDT/FRMT meetings, and collaborations across health services to support release planning and continuity of care.

Qualifications

• Must obtain and maintain LAAC license or WA SW license in good standing to perform duties.
• Apply for agency affiliated counselor license within 7 days of hire; may provide services while processed.

Responsibilities

• Identify transition needs for incarcerated individuals and assist with community resources.
• Complete Reentry Needs Assessment and Health Services Reentry Plan.
• Conduct reentry groups and coordinate with MCO and MDT meetings.
• Coordinate with DDA/DSHS programs and attend multidisciplinary team meetings.
• Input OMNI encounters and document contacts per policy.
• Provide Narcan education and arrange DME for release.
