Job summary

The State of Colorado’s Colorado Department of Corrections is seeking a dedicated case manager to support offender re-entry at Buena Vista Correctional Complex. Using an advocacy-based team approach, you will guide individuals from intake to release and coordinate access to work, education, treatment, and parole plans.

• Three years of experience with a hard-to-serve high-needs client population in corrections or human services.
• Experience with case planning, risk/needs assessment, and coordinating community services.
• Competencies in communication, crisis intervention, and interdisciplinary collaboration.

Responsibilities

• Meets with offenders to guide them per CDOC needs and rules, from intake to release.
• Provides case management using motivational interviewing and CTAP-aligned methods.
• Coordinates access to work, education, treatment, and parole planning.
• Monitors progress, completes assessments, and maintains offender welfare.
• Maintains communication across health, education, and program referrals.
