Home Care Coordinator

We're culturally sensitive, and many of our staff come from within the community that they serve. Operating 24 hours, 7 days a week, we offer reliable, dignified care in your own home and specialize in health services, personal care, and champion services. As a Home Care Coordinator, you will serve as key support for the Operations Manager to manage overall coordination of home care services including scheduling for HHAs and Therapists, assisting with client intake, and other key administrative tasks

Key Tasks Include:

  • Maintain HHAeXchange (home health aide vendor) system, including staffing, billing requests, updating patient demographics coordinating with LHCSA for scheduling, billing and authorization accuracy
  • Main Careficient (patient care) software system including assigning vendors to patient schedule, running reports, updating records for accuracy
  • Organize the scheduling and initiation of home care therapy services for PTs, OTs and SLPs, serve as the primary point of contact for communicating care plans and client progress, and provide ongoing support
  • Handle incoming calls, respond to inquiries, assist with client intake processes, data entry and other administrative tasks as needed

Qualifications:

  • High school diploma; college degree preferred
  • Excellent communication and customer service skills, both written and verbal
  • Tech savvy, with proficiency in Microsoft Office, including Excel
  • Prior experience with HHAeXchange a plus
  • Absolute integrity and sufficient maturity to handle confidential aspects of work
