Family Services, Inc. is seeking a Coordinator for Care Management & Support Services to develop and coordinate programming within our behavioral health program.
You will supervise staff, build partnerships, and help expand services such as community health workers or peer support. The role requires a Bachelor’s degree in a related field (Master’s preferred), plus experience in health home care management, SCN navigation, and peer support models.

Salary: $62,000 - $65,000 annually
Status: Full-Time, Exempt, 40 hours weekly
Supervisor: Chief Program Officer
Schedule: Flexible schedule to meet client needs
The Coordinator for Support Services will develop and coordinate programming that plays a vital role in supporting clients and staff within Family Services’ (FSI’s) behavioral health program, ensuring that Support Services are integrated seamlessly into our system of care. This position fosters partnerships with internal and external stakeholders - including Behavioral Health and Operations leadership, Health Home Care Management teams, SCN navigation professionals, Peer agencies, and other collaborators - to enhance the quality and accessibility of services for those we serve. TheCoordinatorwill supervise program staff and ensure Support Services are high quality and meet all agency and funder goals. TheCoordinatorwill also help prepare FSI for the future, helping the agency to integrateadditionalsupportive services such as community health workers or internal peer support services into our system of care.
