Bright Harbor Healthcare is seeking a Community Integration Coordinator in Toms River, NJ. This full-time role (35 hours/week) focuses on transitioning individuals into the community, supporting recovery, stability, and independence, and coordinating services with community resources.

The position involves supervising program staff, conducting assessments, crisis intervention, discharge planning, and collaboration with housing providers and education programs to benefit consumers.

Qualifications

• Master's degree in Rehabilitation, Health and Human Services or related field.
• Two years of experience working with individuals with severe persistent mental illness.
• Experience in community outreach, social services, or related area.
• Strong interpersonal and communication skills.
• Knowledge of mental health and community resources preferred.
• Valid NJ Driver's License with less than 6 points.

Responsibilities

• Provide clinical and administrative supervision to program staff
• Provide ongoing comprehensive assessments including biopsychosocials and substance use
• Actively participate in performance improvement activities for optimal client care
• Participate in Utilization Review to improve strategies and operations for the benefit of consumer care
• Direct and coordinate housing placement by maintaining working relationships with housing providers
• Maintain relationships with adult education programs and coordinate services for consumers
• Provide crisis intervention as needed
• Supervise discharge planning and referral services.
• Participate in personnel actions including recruitment
• Provide direct clinical care and case management services to clients and their families
