Memorial Hospital of South Bend is seeking a Coordinated Entry Navigator to assist young adults aged 18-24 at risk of homelessness. Description

The Coordinated Entry Navigator is responsible for linking young adults, ages 18‑24, who are at risk for or experiencing homelessness to appropriate services and housing programs within Snohomish County. Based out of the Young Adult Housing department, the Navigators perform case management and resource connection for young adults experiencing homelessness and enrolled in Snohomish County’s Coordinated Entry (CE) system. This includes helping young adults to enroll in the Coordinated Entry system, in addition to other outreach and engagement activities both inside and outside of the agency. The Coordinated Entry Navigator assists with linkage to needed services and institutions and advocates on behalf of the young adults. This includes, but is not limited to: crisis intervention, meeting emergent needs, providing information and advocacy, completing and/or coordinating needs assessments, brokering for services and providing follow‑up with youth. Duties & Responsibilities

Full‑Time 40 hrs/week | On‑site
Wages: $21–$23/hr DOE
This position requires a typical work schedule of Monday‑Friday with some flexibility to work evenings when necessary. Though flexibly scheduled, work weeks will not exceed 40 hours per week.

  • Receive referrals from multiple sources and coordinate with navigator staff, ensuring response within 48 hours. Meet with young adults experiencing homelessness throughout Snohomish County to enroll in Coordinated Entry.
  • Collaborate and partner with other Cocoon House programs, law enforcement, resource centers, schools, churches, and detention centers to identify and target youth who need applicable services.
  • Case‑manage all young adults to connect them to resources for emergent needs, community resources, and housing. Rapidly move youth through caseloads to ensure grant outputs are met.
  • Maintain a cooperative and effective environment free of discrimination, retaliation, and harassment.
  • Collect and enter data to meet all weekly, monthly, quarterly, annual, and other assigned deadlines. Maintain organized, complete files.
  • Attend internal and community meetings and trainings as required.
  • Ensure contract compliance for all activities.

Requirements
Skills needed to be successful in this role
  • A minimum of one year of experience working with young adults experiencing homelessness, or similar populations preferred
  • Effective communication skills.
  • Strong at crisis de‑escalation.
  • Knowledge of issues facing young people, adolescent development, and strengths‑based approaches in working with families.
  • Open commitment to serving, supporting, accepting and collaborating with individuals of all races, ages, genders, backgrounds, political opinions and beliefs, sexual orientations, criminal histories, citizenship or marital statuses and religious beliefs. Special consideration given to applicants with a demonstrated history of supporting and advocating for the LGBTQ community, women, people of color, immigrants or other historically marginalized or persecuted groups.
  • Individuals with lived experience are strongly encouraged to apply.

  • Must be 21 years of age or older
  • DCYF background clearance
  • Acceptable driver's abstract with no more than 2 violations within last 3 years
  • Possess a valid driver's license, auto insurance, and reliable transportation
Physical
  • Ability to effectively communicate with all internal and external clients
  • Be able to hear voices and alarms.
  • Ability to read and comprehend a variety of materials
  • Ability to operate phone, computer, and record information for files and logs
  • Must be able to lift up to 50 lbs
  • Must have ability to run up and down indoor and outdoor stairwells in case of youth emergency.
