Job summary

WCS Healthcare Partners in Melville, NY is seeking a dedicated Coordinated Assessment System Assessor to perform person-centered assessments for individuals with intellectual and developmental disabilities. This role determines service needs and supports NYS programs.

The assessor conducts in-person and virtual evaluations, gathers information from individuals and families, reviews records, and ensures timely documentation while ensuring OPWDD compliance.

Qualifications

  • Bachelor's degree in Social Work, Psychology, Sociology, or related field.
  • Experience conducting comprehensive assessments with I/DD population is desired.
  • Strong interviewing, assessment, and documentation skills.

Responsibilities

  • Conduct in-person and virtual assessments using the CAS tool.
  • Evaluate health status, daily living skills, needs, and support systems.
  • Interview individuals, families, caregivers, and providers.
  • Review medical and social records to support findings.
  • Complete and submit timely documentation in required systems.
  • Coordinate scheduling of assessments and manage caseload.
  • Collaborate with care managers, clinical teams, and providers.
  • Ensure compliance with OPWDD guidelines and quality standards.

Coordinated Assessment System Assessor #26-04789

$22/hour Melville, NY

WCS Healthcare Partners is seeking a dedicated professional to conduct comprehensive, person-centered assessments for individuals with intellectual and developmental disabilities. This role is crucial in determining service needs and ensuring individuals receive appropriate support through New York State programs.

Key Responsibilities
  • Conduct in-person and virtual assessments using the Coordinated Assessment System (CAS) tool
  • Evaluate individuals’ health status, daily living skills, behavioral needs, and support systems
  • Interview individuals, families, caregivers, and providers to gather accurate information
  • Review medical and social records to support assessment findings
  • Complete and submit detailed, timely documentation in required systems
  • Coordinate scheduling of assessments and manage assigned caseload
  • Collaborate with care managers, clinical teams, and service providers
  • Ensure compliance with OPWDD guidelines and quality standards
Requirements
  • Bachelor's degree in Social Work, Psychology, Sociology, or related field
  • Minimum 1 year of experience working with I/DD (intellectual/developmental disability) population
  • Strong interviewing, assessment, and documentation skills
  • Ability to work independently and manage time effectively
  • Proficiency in Microsoft Office and data entry systems
  • Valid NYS Driver’s License or State ID (travel required)

Location: Nassau & Suffolk County, Long Island

Schedule: Full-Time | 5 days x 8 hours

Flexible daytime shifts: 8:30am-4:30pm, 9am-5pm, or 10am-6pm. Must be available 1-2 weekends per month. Assessments may occur any day of the week, including weekends.
